SC@N 360 offers information and referral services, a resource library/computer lab, support groups, and trainings based on the needs and interests identified by our families. Trained parent mentors and SC@N 360 Family Navigators offer time-limited support to help identify and coordinate resources and assist families to navigate the specialized and generic services and entitlements available in Springfield and throughout Massachusetts.
Information and Referral: When someone calls needing information or assistance regarding a family member with a disability, the Enhanced Information and Referral Specialist assesses the needs of the individual or family, links them to appropriate resources, and assists in the completion of identified and necessary applications. SC@N 360 has current and comprehensive information on a variety of topics of interest to persons with disabilities and their families. Staff are familiar with and maintain access to an extensive database of local, state and federal information resources that is user friendly and responsive to the diverse needs of our families.
The Resource Room is available to individuals with disabilities and their families. It contains brochures, manuals, books, and films which can assist an individual or family in learning about a disability or resource. Many families do not have home computers. The computer lab offers these individuals the opportunity to access the Internet, enabling them to do their own research on a topic of interest. We offer family members the opportunity to type and print a letter, resume or other document. Children can come in to use the computers for homework or school projects.Additionally, staff are available to assist those members, who are not familiar with computers, to learn and become more technologically comfortable.
SC@N 360 staff assist families from our priority groups, as defined in the project grant, on a time-limited basis (3 to 6 months), in the development and implementation of an individualized Family Action Plan. Planning approaches build on the strengths and capacities of families and incorporate cultural considerations and beliefs. "Hands-on" assistance actively engages the family in making successful connections with identified programs and services to best address their long-term needs
