Springfield Community Partners: SC@N 360 has established links with five local Community Grant Partners to assist in providing intensive family support planning and case management services to families from the identified priority groups. Families will be able to obtain SC@N 360 services through our Center or any of the five Community Partner Agencies.
 | Lutheran Community Services of Southern New EnglandLCSS began as an orphanage and today is the third largest faith-based provider in New England. LCSS offers a broad range of social services focusing on refugee re-settlement work. In 1999 LCSS began to focus their work with Russian families who have a family member with a disability. LCSS staff are proficient in seven different languages.  | Martin Luther King, Jr Community CenterMLKCC began in 1977 as a Community Fellowship House providing outreach and community ministry. In 1978 the Center established itself as an independent, non-profit organization. The Center works with African American & Latino youth and families offering them opportunities to acquire self- help and self-empowerment skills. Services and supports are also provided for individuals with developmental disabilities and mental health diagnoses and their families. Community Leadership is a hallmark as the Center recognizes the many “Extraordinary People with Extraordinary Talents” within its community.  | Multicultural Community ServicesMCS is a non-profit agency with a proud history of providing a broad range of support services for individuals with developmental disabilities and their families. MCS is the host agency for the Asian Family Support Program, “Hand in Hand”. Hand in Hand’s focus is to identify Asian families who have a son or daughter with a developmental disability and to assist these families in accessing the range of Department of Developmental Services and community services available to everyone within the Commonwealth. Hand in Hand offers Bilingual case Management and Weekend Enrichment Programs.  | New North Citizens’ CouncilNNCC was founded in 1973 in the North End neighborhood of Springfield. As the hub for social, educational and cultural activities for the Hispanic community, NNCC offers intake and referral, work with young parents; a child care center; after school programs; and substance abuse treatment. New North’s community development and advocacy programs support this neighborhood’s desire to be a vibrant part of the Springfield community.  | Springfield Vietnamese American Civic AssociationEstablished in 1992, the mission of SVACA is to support Vietnamese immigrants and refugees in the city of Springfield to become engaged citizens through community empowerment, involvement and education.
